By completing this form, you agree to the following:
Participation
You consent to take part in the Programme and provide your recorded contribution (“the Contribution”).
Rights Granted
You assign all copyright and related rights in the Contribution to the Charity and Media Trust, including
future copyright.
You grant the unrestricted right to use, edit, adapt, translate, copy, add to, or remove material from the
Contribution.
You grant the right to use your name, likeness, biography, photographs, and any materials you provide for
publicity and promotion.
You grant the right to use the Contribution in all media, worldwide, in perpetuity, in all formats now
known or later developed.
You waive all moral rights in the Contribution.
Warranties
You confirm you are entitled to give the Contribution.
The Contribution does not infringe copyright, breach confidence, or contain defamatory material.
Any material you provide is original and you have obtained consent from anyone appearing in it.
All factual statements are true to the best of your knowledge and opinions are genuinely held.
Liability
The Charity and Media Trust are not liable for loss, damage, or injury except where caused by their
negligence.
Data Protection
Personal data will be handled in accordance with data protection law and may be used for journalistic,
artistic, or literary purposes.
A privacy notice is available on request.
General
The Charity and Media Trust is under no obligation to use the Contribution.
This agreement is governed by the laws of England and Wales.
By agreeing to this form, you confirm that you have read and understood all of the above points. You grant
Media Trust the right to use your contribution as part of the film, and you acknowledge that Media Trust
has full editorial control over the final content.

You consent to the child/vulnerable adult taking part in the Programme and providing their recorded
contribution (“the Contribution”).
You understand the nature and purpose of the Programme and their involvement.
You will supervise the child/vulnerable adult at all times and will not leave them in the care of the
production team.

Safeguarding
The Charity and Media Trust will comply with their Child Protection Policy.
The child/vulnerable adult must not exchange personal contact details with anyone involved in
production.
You will notify the Charity and Media Trust if the child/vulnerable adult becomes unwell or may be
contagious.
